This app supports the USDAs Summer Food Service Program (SFSP). It is based on the forms contained in the 2016 Administrative Guide. The meal count form does not track recycled milks or delivery time.
All forms have a place for a signature and upon completion, the form is compiled as a PDF file and can then be emailed.
There are three forms:
1) Pre-operational visit form that is required of all new sites
2) Daily Meal Count form that the sites can use
3) Ethnic and Racial Data Form
